Germany here, "free" like "free lunch"? Well, we pay for it, it gets deducted from our paychecks and it is not a metter of some cents. But it is okay (except that the most wealthy can opt out), still far cheaper than in the US.
Yeah, that map is not very accurate. In Germany for example, people have to pay more and more for all kinds of things since unification. Originally it was all paid for with the monthly deductions, but that has been gradually changing.
Canada's heathcare direct pay costs to individual patients varies by province, and it's definitely not all free. Here in BC for example most people have to pay for all their drug prescriptions, amongst many other things (including eye care and most dental care).
