It's probably quite difficult as it is not possible to get every instance to "relay" messages to a third party to do this type of tracking.
Bluesky has a firehose of all the posts so this type of tracking is much easier as anyone can subscribe to the the stream in various ways like docs.bsky.app/blog/jetstream

It would be extremely easy for them to listen to federated feeds of instances that opt in (or even just mastodon.social), idk what the problems they're talking about are.

Altmetric already tracks an unknown number of personal blogs, it's not clear to me why it would be hard to do it even without a relay - allow instances to sign up to be crawled by altmetric, they crawl the federated feed, simple as.
